Lost Residence Card

HELP!!  I arrived here in Spain at the start of 2012 and registered for my NIE number, Padron, Residence card and SIP card.  I have lost my Residence card and my SIP card (although this is now out of date) - how do I replace them?  I need to register as Autonomo and I can't do this with out the Residence card.  I also need it to apply for a new SIP card.  Can anyone help????
